愛知医科大痛み学講座より The University Center for Pain Medicine and Rehabilitation at Memorial/Hermann Introduction Once pain has been present for a time, and we arbitrarily say six months, it may become centralized and chronic. Centralized means that the nociceptor neurons (pain sensitive nerves) of the spinal cord and brain become altered in some fashion. Even with no pain messages originating from the periphery, the central nociceptors still send signals which the brain interprets as pain. Past experiences and psychosocial stressors all combine to make chronic pain a very different syndrome from acute pain. Because of these factors, the treatment of chronic pain may be less successfully dealt with by medication and nerve blocks than acute pain. Therapy relies more on physical mobilization of the patient, together with teaching him or her better coping mechanisms. However, appropriate medications, diagnostic or therapeutic nerve blocks may be used initially in an attempt to identify pain mechanisms and alter the pain cycle.
